CONGRATULATIONS: you’ve taken a big, brave step in managing your diabetes. When you are in the Care Circle, you’re stronger than your illness. You can know that an active, full life is within your reach. You’re about to become an expert on your body and a hero to yourself and your family.

EVERY STEP OF THE WAY, YOU’LL HAVE BACKUP.
Your doctor, nurse practitioner, family, friends, and this program’s coordinators are now your Care Circle. We’re here to answer questions and give thumbs up. We’ll help you meet challenges and give you a toolkit to monitor your body’s changes and improvements. And when you achieve your success, we’ll be celebrating, too, because our goal is your health.

Self-Management
SKILLS

Managing your health is the most powerful tool you’ll be given in this program. You’ll pick it up gradually, and your Care Circle will be here to hold your hand as you learn the first steps. 

Then like any new skill, listening to your body and making smart choices will get easier the more you do it. You’ll have more energy, enjoy life more, and feel more positive about your future. Before you know it, you’ll be standing on your own, enjoying the confidence that comes with saying “I did it!” Here are some tips to help you get there:

  • Find an exercise you enjoy doing, so you’ll keep doing it.

  • Use the Kitchen Keeper to choose foods that support your health.

  • Keep appointments with your doctor and get tests and screenings on time.

  • Write your blood sugar levels, medications, meals, and exercise each day in your journal.

  • Share with your Care Circle when you reach your goals and when you’re struggling.

  • Make time for regular self-care to manage stress, which can raise blood sugar levels.

  • Have your Project ALTO-D Booklet close by, to better understand and “Learn About Diabetes” – including symptoms, self care ideas, and simple exercises you can do at home to feel better.
