Rosa Hernandez
COMMUNITY HEALTH WORKER
Rosa Hernandez is a certified community health worker (CHW) at San Fernando Community Health Center. Her mission is to empower individuals to take control of their wellbeing and foster a healthier, more equitable community for all.
Hernandez believes her compassion, dedication and experience have uniquely qualified her to serve in the healthcare industry. Hernandez’ extensive educational training and her internship as a CHW at Charles R. Drew University School of Medicine and Science have thoroughly prepared her to work with patients in clinic-based settings.
Additionally, Hernandez has over three years of experience working in nonprofit community organizations that include Pacoima Beautiful, a community improvement organization, and Youth Speak Collective, a youth-driven leadership training program that pairs high school students with business and community professionals. Hernandez has also worked at the Los Angeles Unified School District as an academic assistant educator.
Hernandez holds a Bachelor of Arts in sociology from California State University, Northridge and an associate degree in health science.
